This! This is me. I had to break a 48 hour fast because I couldn’t deal with the heartburn any longer. It immediately went away once in got some food down. I thought it would go away once I woke up in the morning but it got worse. I was strictly drinking water and one seltzer. I’ve been searching for reasons and solutions because it was SO bad.
Disclaimer; I’m not a doctor, so I don’t know if there are unwanted side effects from this solution. Also, re-posting this comment from another thread where I originally wrote it.
I’ve also dealt with heartburn issues when fasting, and it kind of turned me off to it for a while. I would experience it after just a day or two. I recently started taking omeprazole (OTC) at the start of a fast and it’s reduced that heartburn feeling tremendously (have taken it for 3 days of a 4 day fast three times now). I will also munch on an antacid tablet if I start to feel anything - to keep that unpleasantness at bay. I think the antacid probably messes up my insulin levels a little bit, but if it allows me to fast longer I think it’s an overall benefit.
Sidenote – I also have noticed that breaking my last three fast has been a lot more pleasant after taking the omeprazole. No diarrhea, loose stool, or gassiness when re-feeding
I’m doing OMAD diet, so everyday after 24 hours of fast, When I eat my meal, before the meal I started taking ACV (Apple cider vinegar) diluted in water before consuming meal. my acid reflux is gone.
I saw some video on youtube about it and Tried it.
just search ACV acid reflux and you will find many videos having benefits in acid reflux.

I used to get a lot of heartburn and AR. Since I started eating a lot more large green salads, no problems. I used to get ‘gut raunch’ when I didn’t eat, so I make sure I eat a big salad or veggies before I fast and I haven’t had any issues.
